Submission Forms

Choose your appropriate submission form below. All forms are available as PDF only. You can download Adobe Reader here. You may download and print as many copies of the forms you need.

All submissions must be original and created by the student.
One submission per student per category.

Literary Submissions

Literary submissions can be mailed or hand-delivered, or sent via email, PDF, RTF or DOC by February 27, 2009.

Literary submissions will not be returned. The SLHC will present all literary submissions in binders throughout the exhibit, organized by grade and school level.

Students submitting works of poetry and prose may be given the opportunity to perform a reading during the opening reception depending on the number of submissions. Please download the required submission form available above. Depending on the number of submissions, a limited number of students will be permitted to perform a reading to be selected by SLHC staff. The schedule of performances for Sunday, March 15, 2009, and number of performances will be determined by number of poetry/prose submissions and order of receipt of work with required submission form.

The teacher or homeschool parent will be notified by SLHC staff by March 2, 2009, when the student has been scheduled to perform.

Art Submissions

Art submissions must be accompanied by a short paragraph on student submission form containing no more than 250 words describing their creative process and correlation to folklore, fantasy, or mythological inspiration. Space is provided for their artist statement on the back of the artwork submission form.

Our Open Studio Day
with Stacy Clark

Your art studio for the day!

Come to the Heritage Center on Saturday, January 17, 2009, 10am-3pm to meet Stacy Clark, learn about her techniques and influences, exchange project ideas, and work on your entry under the guidance of Stacy Clark and our museum educator (supplies and refreshments provided!). Designed especially for students with limited art room access. More information.

2D submissions may include storyboards, comic strips, prints and drawings, oil/acrylic painting, and digital submissions. All digital submissions must be submitted in print form.

3D submissions may include but are not limited to fiber arts, art dolls, clay and wire sculpture, dioramas, puppets, cloth and felt books.

Most media will be accepted, however all submissions must be dry to touch upon delivery to SLHC. Assembly required of 3D works must be completed before arrival to SLHC.

All Submissions

All submissions must include student’s first name for identification. Only one submission per student per category (i.e. poetry, prose, 3D, 2D, etc) will be permitted. Acceptance of student literary and art submissions will begin on February 6, 2009, and run through February 27, 2009. Submission form must be attached to artwork or literary submission with removable adhesive (form will be removed for exhibition).
